I am looking forward to learning about this event. Where was the FIRE PROTECTION ENGINEERING, that would correctly design the mitigation, provide the criteria and requirements for acceptance and ITM?
Sometimes, yes, the Swiss cheese lines up to have a bad one, but there is more to the story here.
Were drawings and designs developed by a non-FPE trying to code jockey? Were hazards and capabilities being addressed (system capabilities and hazard of contents with management being applied/implemented correctly and consistently)?
